Please make sure to provide the following 

• Amount requested and purpose of the loan
   
• Applicant information (name, address, phone, SSN, date of birth)
   
• Housing details (own/rent, monthly mortgage or rent, utilities included)
   
• Employment & income (employer, years of service, gross income)
   
• Other income (source and gross amount, if you want it considered)
   
• Signature & date authorizing us to process your application
  Attach income verification for all income you want considered.
   

Optional protection: You may select Debt Protection options (Life Plus; Life Plus & Disability; Life Plus, Disability & Involuntary Unemployment) or choose No Protection. Coverage is optional and does not affect credit decisions.
